Design Themes Modification When Updating from CNCat 4.1.2 to CNCat 4.1.3

Modifications:

• The old templates for expanded fields have been removed.
• One new template for the expanded fields has been added.

New Elements:

• Template add_extfield

Apply Modifications:

1. Delete all templates which names start with extfield_ext
2. Copy add_extfield template from the new version of add.tpl file
